1. PURPOSE 目的
This procedure provides the minimum requirements for the safe operation of forklift trucks. It enhances the safety of all personnel involved in the operation of forklift trucks, and also others who may be working in close proximity to forklift trucks. 本程序提供最基本的叉車安全操作要求。旨在提高叉車操作人員以及叉車附近其他工作人員的安全性。
2. SCOPE 范圍
This procedure applies to all forklift truck operations on the terminal regardless of the ownership of the forklift trucks. 本程序適用于庫區(qū)范圍內(nèi)所有叉車操作(無論何方所有)。
3. DEFINITION 定義
3.1. Qualified Forklift Operator is an operator who has undergone the required training on forklift operations and has the license issued by the interrelated authority. 合格叉車操作人員指經(jīng)相關(guān)叉車操作培訓(xùn)并擁有相關(guān)管理部門頒發(fā)的證書的操作人員。
3.2. Flammable Liquids refer to products with flash point below 37.8OC. 易燃液體指閃點(diǎn)低于37.8OC的產(chǎn)品。
4. RESPONSIBILITIES 職責(zé)
4.1. It is the responsibility of the forklift operator to conform to the stipulated safety requirements on use of forklift. 叉車操作人員有責(zé)任遵守與叉車使用相關(guān)的安全要求。
4.2. It is the responsibility of the line managers / supervisors to ensure that only trained operators are authorized to operate a forklift. 直接主管負(fù)責(zé)確保:只有經(jīng)過培訓(xùn)的有資質(zhì)的人員才能操作叉車。
They shall also ensure that the forklift supplied by contractor are periodically maintained and serviced. If the forklifts are to be used for handling of flammable liquid in drums, they should be installed with spark arresting devices such as arrestors, flame arrestor, air-start device, etc. In short, these forklifts shall be designed for use in Class 1 Division 2 (or Zone 2) Classified Hazardous Areas (refer to SHE-SWP-16). 并應(yīng)確保承包商提供的叉車接受定期維修和保養(yǎng)。若叉車用于桶裝易燃液體搬運(yùn),應(yīng)安裝阻火設(shè)備如阻火器、火焰防止器、氣動(dòng)裝置等。簡而言之,叉車應(yīng)適用于《危險(xiǎn)區(qū)域分類》1級(jí)2分區(qū)(參見SHE-SWP-16)。
4.3. Contractor who supplies the forklifts is responsible to make the necessary arrangements for any forklift repair or periodic servicing and inspection by the manufacturer so as to upkeep the safe operability of the forklift trucks. 提供叉車的承包商負(fù)責(zé)就制造廠家叉車維修或定期保養(yǎng)、檢查服務(wù)進(jìn)行必要的安排,以維持叉車的安全可操作性。
5. SAFETY REQUIREMENTS 安全要求
5.1. All forklift trucks must only be operated by qualified operators. 只有有資質(zhì)的人員才能進(jìn)行叉車操作。
5.2. Forklift trucks should be suitable for use in Class 1 Division 2 (or Zone 2) Classified Hazardous Areas if they are used for handling of flammable liquids in drums. The maximum exhaust surface temperature should be about 200OC (IEC T-Code). 若用于桶裝易燃液體搬運(yùn),則叉車應(yīng)適合在《危險(xiǎn)區(qū)分類》1級(jí)2分區(qū)(參見SHE-SWP-16)中使用。最大表面釋放溫度應(yīng)為200OC(IEC T-編碼)。
5.3. All traffic rules and warning signs on the terminal are to be strictly obeyed. 嚴(yán)格遵守庫區(qū)所有交通規(guī)則和警示標(biāo)記。
5.4. If there is a need to operate a forklift in hazardous areas, other than the roadways and Drum Yard / Warehouse, the appropriate Hot Work Permit must be obtained from CCR. 若有必要在危險(xiǎn)區(qū)內(nèi)而非道路上或桶堆放場/倉庫內(nèi)進(jìn)行叉車操作,則必須從中控室取得熱作業(yè)許可證。
5.5. Load limits of the forklift trucks must be stenciled on the mast of each truck where it is clearly and readily visible to the operator. 叉車荷載極限必須印在叉車升降架上方以便操作人員清楚看到的位置。
5.6. Before operating the forklift truck at the beginning of the day, the operator must inspect the forklift truck to verify that it is in a good and safe working conditions. 在每天開始叉車操作前,操作人員必須對(duì)叉車進(jìn)行檢查,以確定其處于良好、安全的工作狀態(tài)。
5.7. The operator must be alert and cautious when operating the forklift truck. He must watch for other vehicles, equipment, obstacles, road surface conditions and pedestrians. 操作人員在叉車操作時(shí)必須保持警覺和謹(jǐn)慎。操作人員必須注意其它車輛、設(shè)備、障礙、路面狀況和行人。
5.8. No passenger is allowed to ride on the forklift truck. 叉車不許搭載乘客。
5.9. The operator shall face in the direction of travel. 操作人員應(yīng)面對(duì)行駛方向。
5.10. The operator shall keep the mast tilted back with forks 150 to 300 mm above the ground while traveling loaded or unloaded. 行駛中無論有無負(fù)重,操作人員應(yīng)將前叉后傾,車叉距離地面150至300 mm。
5.11. The operator shall check for adequate overhead and lateral clearance before entering and during subsequent maneuvering in any building, plant facility or congested area. 在進(jìn)入任何建筑、裝置設(shè)施或密集區(qū)域之前及之后操作中,操作人員應(yīng)檢查上方及側(cè)面距離是否足夠。
5.12. The operator shall sound the horn at junctions, corners and in areas where vision is being obstructed. 在交叉口、轉(zhuǎn)彎處以及視線受阻區(qū)域內(nèi), 操作人員應(yīng)鳴喇叭。
5.13. The operator shall drive slowly and cautiously in the following circumstances. 在以下情況下,操作人員應(yīng)緩慢、謹(jǐn)慎駕駛:
when making a turn 轉(zhuǎn)彎時(shí)
when reversing 倒車時(shí)
driving over a hump 凹凸不平地面行駛時(shí)
driving up or down a ramp 上下坡道時(shí)
driving on a wet, slippery or uneven surface 濕、滑、不平地面行駛時(shí)
driving in narrow roads 狹窄路面行駛時(shí)
driving in high noise areas or where vision is obstructed 高噪音區(qū)或視線受阻區(qū)域內(nèi)行駛時(shí)
driving in close proximity to potentially hazardous area 危險(xiǎn)區(qū)附近行駛時(shí)
5.14. The operator shall travel in the reverse mode if the load being carried obstructs the forward view. 若所載重物阻礙前方視線,操作人員應(yīng)倒行。
5.15. The operator must always ensure that load limits are not exceeded. He shall not carry a load if it is not properly secured or if it extends above the carriage backrest unless no part of the load can possibly slide back towards the operator.操作人員必須確保不超出荷載極限。若重物未適當(dāng)固定或伸出貨物安全架,操作人員不得承運(yùn),重物任一部分沒有向后滑向操作人員的可能時(shí)除外。
5.16. The operator shall avoid making sudden stops/start and also, running over loose objects and holes which may be present in the path of travel. 操作人員應(yīng)避免急停急起,并避免在有松軟物體或有坑的路面上行車。
5.17. Forklift truck shall not be used as a working platform for people. It shall not be used for any purpose other than that for which it was designed. 叉車不得用作人員工作平臺(tái)。除設(shè)計(jì)目的外,不得將叉車用于其它用途。
5.18. No personnel is allowed to stand or walk under the elevated portion of the forklift truck whether loaded or not. 無論有無重物,任何人不得在叉車抬高部分下站立或行走。
5.19. Collisions, damages or near misses shall be reported to the Shift leader immediately. 若有碰撞、損害或未遂事故,應(yīng)立即報(bào)告當(dāng)班領(lǐng)班。
5.20. No forklift operation is allowed on flatbed trailers or lorries, except for stuffing of containers. 叉車不得在平板拖車或卡車上進(jìn)行操作,集裝箱裝貨時(shí)除外。
5.21. In case of an incident involving spillage of flammable liquid, the operator should attempt to switch-off the forklift engine and notify CCR. 若易燃液體泄漏引發(fā)事故,操作人員應(yīng)盡量關(guān)閉叉車發(fā)動(dòng)機(jī),并通知中控室。
5.22. Prolong operation of engine-driven forklift trucks in poorly ventilated areas must be avoided. 必須避免內(nèi)燃機(jī)叉車在通風(fēng)不良區(qū)域內(nèi)長時(shí)間作業(yè)。
5.23. Whenever left unattended, the operator shall park the forklift truck in a location that does not obstruct safety or fire protection equipment and roadways. The following actions need to be done. 離開叉車前,司機(jī)應(yīng)將叉車停在對(duì)安全或消防設(shè)備與通道無妨礙的位置。需采取以下措施:
a. the forks lowered and tilted forward so that the fork tips touch the ground. 降下前叉,以便前叉能接觸地面。
b. the functional control in neutral mode 控制開關(guān)在空檔狀態(tài)。
c. the parking brake engaged 使用停車制動(dòng)。
d. the ignition or key switch turned off 關(guān)閉點(diǎn)火或鑰匙開關(guān)。
5.24. While operating the forklift truck, the operator must put on safety shoes. Hearing protection device needs to be worn too. 進(jìn)行叉車操作時(shí),操作人員必須穿上安全鞋,并使用必要的聽力保護(hù)用品。
5.25. Hoses may be carried by a forklift truck if they are properly enclosed within a pallet box which is secured to the forklift truck. Carriage of hoses hanging from the forks is extremely unsafe and is not allowed.如果使用叉車運(yùn)輸軟管,使用可固定的托盤來運(yùn)輸是安全的。不允許使用叉車直接運(yùn)送軟管。
6. BATTERY CHARGING AND REFUELING 充電及加油
6.1. Re-charging of batteries for electric forklift trucks must be done in an approved area where adequate safety and ventilation facilities are available. 電動(dòng)叉車必須在指定區(qū)域內(nèi)進(jìn)行電池充電。指定區(qū)域應(yīng)具備足夠的安全與通風(fēng)設(shè)施。
6.2. Refueling of engine-driven forklift trucks must be done at the fuel dispersing area, with the engine switched-off. 內(nèi)燃機(jī)叉車必須在加油區(qū)進(jìn)行加油,且加油時(shí)關(guān)閉發(fā)動(dòng)機(jī)。
7. MAINTENANCE AND INSPECTION 維修與檢查
Contractor who supplies the forklifts will ensure that all forklift trucks are repaired when necessary, and also inspected and service by the manufacturer periodically. Maintenance records will be filed and kept accordingly. 提供叉車的承包商將確保叉車制造廠商對(duì)所有叉車進(jìn)行必要的修理、定期檢查和保養(yǎng)。維修記錄要存檔、保存。
